faster implementation of pread_iolist. Closes COUCHDB-762
Why it's faster:
- 2 pread() calls (instead of 3) for MD5-prefixed terms
- 1 gen_server:call (instead of 3) in all cases, also guarantees that
we don't intersperse pread() calls to different parts of the file when we
have concurrent readers.
